Notice of Voluntary Conversion to Chapter 7  

1. Select Bankruptcy.

2. Select Notices.

3. Ensure correct case name and number match the document you are filing.

4. Select Document Type: Notice of Voluntary Conversion to Chapter 7.

5. Select party filer.

   • Party not listed, Add/Create New Party.

6. Browse, verify and attach Notice (PDF file.)

   • Attach supportive documents if applicable.

7. Enter statistical data related to conversion.

   • New Chapter - 7

   • Asset Notice - No

   • Date converted - current date.

8. Enter "cc" in credit card field.  Court will charge your card and send you receipt.

9. Modify text, if applicable.

10. Verify final docket text before submission.
